Abstract

This study presents AI-HEART, a cloud-based information system for managing and analysing long-duration ambulatory electrocardiogram (ECG) recordings and supporting clinician decision-making. The platform operationalises an end-to-end pipeline that ingests multi-day three-lead ECGs, normalises inputs, performs signal preprocessing, and applies dedicated deep neural networks for wave delineation, noise/quality detection, and beat- and rhythm-level multi-class arrhythmia classification. To address class imbalance and real-world signal variability, model development combines large clinically annotated datasets with expert-in-the-loop curation and generative augmentation for under-represented rhythms. Empirical evaluation on three-lead ambulatory ECG data shows that delineation accuracy is sufficient for automated interval measurement, noise detection reliably flags poor-quality segments, and arrhythmia classification achieves high specificity with clinically useful macro-averaged performance across common and rarer rhythms. Beyond predictive accuracy, AI-HEART provides a scalable deployment approach for integrating AI into routine ECG services, enabling traceable outputs, audit-friendly storage of recordings and derived annotations, and clinician review/editing that captures feedback for controlled model improvement. The findings demonstrate the technical feasibility and operational value of a noise-aware AI-ECG platform as a digital health information system.

HeartBeatAI: An Interpretable and Robust Deep Learning Framework for Multi-Label ECG Arrhythmia Detection

While Deep Learning (DL) enhances automated electrocardiogram (ECG) analysis, clinical deployment is hindered by class imbalance and the generalization gap. This paper presents HeartBeatAI, a deep learning framework combining domain generalization, multi-scale feature aggregation, and clinical explainability for robust 12-lead ECG classification. Moving beyond image-based paradigms, HeartBeatAI integrates a Squeeze-and-Excitation (SE) ResNet to isolate diagnostic leads alongside a Multi-Layer Concentration Pipeline to capture macro-rhythm and micro-morphological anomalies. To mitigate domain shift, the framework employs MixStyle regularization and Label Smoothing. Rigorous benchmarking across four large-scale datasets using intra-source and Leave-One-Domain-Out (LODO) protocols demonstrates high performance (98% Macro F1-score) under intra-source conditions. However, LODO evaluations reveal significant degradation in detecting rare anomalies, highlighting a persistent challenge in cross-institutional deployment.

DeepArrhythmia: Segment-Contextualized ECG Arrhythmia Classification via Selective Evidence Acquisition

Beat-level Electrocardiography (ECG) arrhythmia detection aims to assign an arrhythmia class to each beat in a recording, yet many existing systems treat beats as isolated local instances. This is limiting because beat labels often depend on multi-beat rhythm context, including timing, compensatory pauses, and beat-to-beat morphological consistency. We present DeepArrhythmia, a tool-grounded multimodal framework for segment-contextualized beat-level ECG arrhythmia classification. Given a multi-beat ECG segment, DeepArrhythmia combines the raw ECG signal and a rendered waveform image, localizes R peaks to identify beat instances, and produces structured beat-level predictions. The framework decouples physiological measurement from evidence integration using specialized tools for beat localization, numerical rhythm--morphology extraction, and morphology-focused textual analysis. DeepArrhythmia uses segment-level confidence to route between minimal and rich evidence states, since richer physiological evidence is not uniformly useful. This agentic design integrates rhythm context, explicit physiological grounding, and selective evidence acquisition for decision making.

Wearable ECG Quality Assessment: A Deep Learning and Ambulatory Context-Awareness Approach

This paper presents and evaluates a Deep Learning-based (DL-based) Signal Quality Assessment (SQA) model to distinguish between clean and noisy ambulatory Electrocardiograms (ECG). The model is trained on Copenhagen Center for Health Technology-Contextualized Arrhythmia Database (CACHET-CADB), which, to the best of our knowledge, is the first ambulatory ECG database with both physical and patient-reported contextual data. The model shows stable performance on different databases such as MIT-databases and the latest PyhsioNet/Cinc Challenge 2021 databases. Subsequently, the paper demonstrates how complicated ECG noise can be investigated by the SQA model and the physical contextual data.
