Abstract

Multimodal clinical AI is limited by weakly aligned inputs and the absence of domain-specific interpretable representations, particularly when learning from dense video stream, structured time-series, and template-based kinematic text. Here we present ScoliDetect, an explainable framework for adolescent idiopathic scoliosis screening from monocular gait video, built around a kinematic knowledge map (KKM) and complementary template-based kinematic text derived from per-sequence pose statics. KKM is a fixed-index structured representation that encodes gait features across absolute motion, self-skeleton configuration and joint-joint signal correlation, providing anchor-referenced multimodal fusion and factor-level interpretation. We integrate video, KKM, and template-based kinematic text through bidirectional cross-attention with latent-bottleneck aggregation. In a multicenter cohort (n = 1,858 after exclusions), prespecified supervised ablations on an external screening cohort show that KKM-mediated multimodal fusion outperforms unimodal models and late concatenation. Under a staged training protocol, trimodal contrastive pretraining is applied after architecture selection as representation initialization, improving external ROC-AUC from 0.961 to 0.972. Furthermore, the structured nature of the KKM provides inherent, factor-level attributions mapped directly to specific kinematic phases and skeletal indices, offering verifiable interpretability. The results demonstrate that embedding explicit structural topologies into latent spaces significantly enhances both the generalization and explainability of multimodal pattern analysis systems.

TokenSTFormer: A Tokenized Spatial-temporal Attention Model for Holistic Motion Analysis in Adolescent Idiopathic Scoliosis Screening

Adolescent Idiopathic Scoliosis (AIS) is a prevalent spinal deformity in adolescents that, if left untreated, can result in severe health outcomes. Traditional screening methods are limited by subjective interpretation, reliance on professional expertise and low scalability. To address these challenges, we present ScoliGait dataset, which comprises 1,516 gait video clips paired with corresponding X-ray records. We also introduce TokenSTFormer, a novel model that tokenizes spatial and temporal semantics to enhance feature representation and convergence. Our model achieves state-of-the-art performance, surpassing vanilla Vision Transformer encoder across key metrics, including accuracy of 0.79. This study highlights the potential of leveraging holistic motion features derived from gait video and attention-based models for scalable, cost-effective AIS screening, paving the way for future clinical applications in scoliosis detection.

BioGait-VLM: A Tri-Modal Vision-Language-Biomechanics Framework for Interpretable Clinical Gait Assessment

Video-based Clinical Gait Analysis often suffers from poor generalization as models overfit environmental biases instead of capturing pathological motion. To address this, we propose BioGait-VLM, a tri-modal Vision-Language-Biomechanics framework for interpretable clinical gait assessment. Unlike standard video encoders, our architecture incorporates a Temporal Evidence Distillation branch to capture rhythmic dynamics and a Biomechanical Tokenization branch that projects 3D skeleton sequences into language-aligned semantic tokens. This enables the model to explicitly reason about joint mechanics independent of visual shortcuts. To ensure rigorous benchmarking, we augment the public GAVD dataset with a high-fidelity Degenerative Cervical Myelopathy (DCM) cohort to form a unified 8-class taxonomy, establishing a strict subject-disjoint protocol to prevent data leakage. Under this setting, BioGait-VLM achieves state-of-the-art recognition accuracy. Furthermore, a blinded expert study confirms that biomechanical tokens significantly improve clinical plausibility and evidence grounding, offering a path toward transparent, privacy-enhanced gait assessment.

Sketch It Out: Exploring Label-Free Structural Cues for Multimodal Gait Recognition

Gait recognition is a non-intrusive biometric technique for security applications, yet existing studies are dominated by silhouette- and parsing-based representations. Silhouettes are sparse and miss internal structural details, limiting discriminability. Parsing enriches silhouettes with part-level structures, but relies heavily on upstream human parsers (e.g., label granularity and boundary precision), leading to unstable performance across datasets and sometimes even inferior results to silhouettes. We revisit gait representations from a structural perspective and describe a design space defined by edge density and supervision form: silhouettes use sparse boundary edges with weak single-label supervision, while parsing uses denser cues with strong semantic priors. In this space, we identify an underexplored paradigm: dense part-level structure without explicit semantic labels, and introduce SKETCH as a new visual modality for gait recognition. Sketch extracts high-frequency structural cues (e.g., limb articulations and self-occlusion contours) directly from RGB images via edge-based detectors in a label-free manner. We further show that label-guided parsing and label-free sketch are semantically decoupled and structurally complementary. Based on this, we propose SKETCHGAIT, a hierarchically disentangled multi-modal framework with two independent streams for modality-specific learning and a lightweight early-stage fusion branch to capture structural complementarity. Extensive experiments on SUSTech1K and CCPG validate the proposed modality and framework: SketchGait achieves 92.9% Rank-1 on SUSTech1K and 93.1% mean Rank-1 on CCPG.
