MedP-CLIP: Medical CLIP with Region-Aware Prompt Integration

Abstract

Contrastive Language-Image Pre-training (CLIP) has demonstrated outstanding performance in global image understanding and zero-shot transfer through large-scale text-image alignment. However, the core of medical image analysis often lies in the fine-grained understanding of specific anatomical structures or lesion regions. Therefore, precisely comprehending region-of-interest (RoI) information provided by medical professionals or perception models becomes crucial. To address this need, we propose MedP-CLIP, a region-aware medical vision-language model (VLM). MedP-CLIP innovatively integrates medical prior knowledge and designs a feature-level region prompt integration mechanism, enabling it to flexibly respond to various prompt forms (e.g., points, bounding boxes, masks) while maintaining global contextual awareness when focusing on local regions. We pre-train the model on a meticulously constructed large-scale dataset (containing over 6.4 million medical images and 97.3 million region-level annotations), equipping it with cross-disease and cross-modality fine-grained spatial semantic understanding capabilities. Experiments demonstrate that MedP-CLIP significantly outperforms baseline methods in various medical tasks, including zero-shot recognition, interactive segmentation, and empowering multimodal large language models. This model provides a scalable, plug-and-play visual backbone for medical AI, combining holistic image understanding with precise regional analysis.

MedSIGHT: Towards Grounded Visual Comprehension in Medical Large Vision-Language Models

Medical large vision-language models (Med-LVLMs) have recently achieved remarkable progress in vision-language comprehension and medical image segmentation. However, existing models still struggle to unify these two capabilities, which is essential for achieving clinically reasoning that connects visual findings with semantic interpretation. We present MedSIGHT, a unified framework that equips Med-LVLMs with structured, pixel-level understanding for grounded visual comprehension. MedSIGHT introduces a novel Region Perceiver module that produces region-centric tokens, encoding spatial information directly into representation space of the language model. We further propose a medical region codebook into the LLM vocabulary, allowing the model to generate discrete region codes as symbolic representations of anatomical and pathological regions. These codes are decoded through the Region Perceiver to reconstruct segmentation mask, achieving end-to-end spatial grounding. Lastly, MedSIGHT combines Region Perceiver, Codebook and LLM using our proposed progressive training strategy to gradually aligns these modules stably. Trained on only 72K multimodal instruction pairs, MedSIGHT achieves state-of-the-art performance across diverse imaging modalities on both medical comprehension and segmentation tasks.

RoiMAM: Region-of-Interest Medical Attention Model for Efficient Vision-Language Understanding

Vision-Language Models (VLMs) facilitate medical visual question answering (MedVQA) by jointly interpreting images and text. However, existing models typically depend on large architectures and closed-set answers, which limits their efficiency and potential clinical applicability. To overcome these shortcomings, we introduce RoiMAM, an efficient VLM. It integrates a training-free ROI Generation Module with Semantic Selective Suppression to focus on lesion-relevant regions, alongside a Text Prompt Enhancer module that provides modality-specific context without introducing training parameters. Compared to the widely used MedVInT-TD model, our design achieves efficient and accurate diagnosis at less than 20% of the model size, while improving accuracy by approximately 2% on SLAKE and 4.6% on PMC-VQA.

MedUP: Awakening Unified Understanding and Perception in Medical Vision-Language Models

Medical Vision-Language Models (Med-VLMs) excel at verbalizing visual content, yet precise visual perception, segmentation, and grounding remain challenging. Existing approaches either verbalize regions as coordinate strings or rely on external modules that decouple perception from understanding, creating representation gaps for region-language alignment. We present MedUP, a Med-VLM that natively unifies perception and understanding within a shared token space. At its core lies UniMedTok, a region tokenizer that encodes masks as discrete tokens in the LLM vocabulary, enabling the model to seamlessly interleave mask tokens with text. We curate UniMed-Train, a 1.84M-instance corpus spanning text-guided segmentation, region-grounded understanding, medical VQA and CoT-based segmentation, and introduce UniMed-Bench for unified evaluation. Extensive experiments show that MedUP outperforms native, agentic, and dual-decoder Med-VLMs across all tasks while remaining competitive with specialist segmentors, demonstrating the strong potential of unified understanding and perception modeling.
