EndoGSim: Physics-Aware 4D Dynamic Endoscopic Scene Simulations via MLLM-Guided Gaussian Splatting

Abstract

In robot-assisted minimally invasive surgery, high-fidelity dynamic endoscopic scene reconstruction and simulation are crucial to enhancing downstream tasks and advancing surgical outcomes. However, existing methods primarily focus on visual reconstruction, lacking physics-based descriptions of the scene required for realistic simulation. We propose a unified framework that achieves physics-aware reconstruction and physical simulation of endoscopic scenes through Multi-modal Large Language Models (MLLMs)-guided Gaussian Splatting. Our approach utilizes 4D Gaussian Splatting (4DGS) integrated with pre-trained segmentation and depth estimation to represent deformable tissues and tools. To achieve automatic inference of physical properties, we introduce an object-wise material field that initializes material parameters via MLLM and refines them through a differentiable Material Point Method (MPM) under joint supervision from rendered images and optical flow. Validated on both open-source and in-house datasets, our framework achieves superior simulation fidelity and physical accuracy compared to state-of-the-art methods, underscoring its potential to advance robot-assisted surgical applications.

Accelerated Spatio-Temporal Gaussian Rendering via Kinematic and Semantic Priors

Reconstructing dynamic 3D scenes from multi-view videos is a foundational task for robotics, AR/VR, and digital twins. While 3D Gaussian Splatting (3DGS) provides state-of-the art scene representation, its dynamic adaptations (4DGS) face significant challenges. Early MLP-based deformation models excel at interpolating within training frames but fail at temporal extrapolation --- important feature for forecasting in predictive systems. To address this, physically grounded approaches have been proposed to enable robust future forecasting, while, concurrently, semantic 4DGS methods have been developed for scene analysis and language-driven prompting. However, there is currently no framework that unites both physical consistency and semantic understanding. In this paper, we propose the first, to the best of our knowledge, unified 4DGS approach that is both physically grounded and semantically aware. Furthermore, to optimize computational efficiency, we introduce a novel rigid-body constraint mechanism. Instead of predicting the physical dynamics for every Gaussian particle, our model predicts the motion of a single anchor Gaussian per semantic object, updating the remaining Gaussians via relative transformations. This architectural design significantly accelerates novel view synthesis compared to an unconstrained baseline, while enabling object-accurate extrapolation and intuitive semantic interaction. DeGenseGS: Geometrically and Semantically Decoupled Surgical Scene Understanding in 4D Gaussian Splatting

Real-time, text-promptable 4D reconstruction is indispensable for autonomous surgical interaction. Severe misalignment between semantic meaning and physical anatomy still persists, largely because existing solutions integrate Vision-Language Models into deformable fields via a rigid coupling scheme that tightly binds semantic features to geometric warping. In this paper, we propose DeGenseGS, Geometrically and Semantically Decoupled Surgical Scene Understanding in 4D Gaussian Splatting, a novel framework that independently models semantic evolution and geometric deformation. Specifically, we propose a HexPlane-based spatiotemporal entanglement module that uses shared kinematic latents to synchronize semantic mutations with scene dynamics, while explicitly disentangling semantic updates from geometric deformation. To further ensure robustness against reconstruction artifacts, we devise a Rasterization-Native Semantic Extraction mechanism that infers semantics from topologically continuous feature maps. Additionally, we incorporate an angular-aligned optimization strategy that conforms to the native hyperspherical latent space, thereby preventing semantic distortion. Extensive evaluations on the CholecSeg8k and EndoVis18 datasets demonstrate that DeGenseGS achieves state-of-the-art performance. Our framework yields enhanced geometric completeness and robust semantic-anatomic alignment, enabling spatially continuous segmentation despite drastic tissue deformation and topological transitions.

GASE: Gaussian Splatting-Based Automated System for Reconstructing Embodied-Simulation Environments

Training embodied agents in the real world requires skilled operators and expensive hardware. Simulation environments offer a compelling alternative by enabling large-scale, cost-effective data augmentation. Consequently, rapidly constructing high-fidelity simulation scenes with a minimal sim-to-real gap has become a critical objective in robot learning. While reconstruction-based methods provide superior visual quality, current workflows are hindered by inefficient data acquisition and subpar foreground object extraction. We thus propose GASE, a highly automated system for simulation scene construction. GASE leverages multi-view video streams from panoramic camera arrays to enable rapid environment scanning. To ensure high-quality asset generation, our pipeline introduces a camera-pose-based strategy that robustly extracts objects across frames in the 2D domain, followed by high-fidelity scene inpainting. Foreground objects and the static background are then reconstructed independently and seamlessly imported into physics simulators for policy training. Extensive experiments demonstrate that GASE outperforms existing 3D Gaussian-based methods in segmentation accuracy by over 10% while achieving state-of-the-art inpainting quality. Furthermore, real-robot deployments across manipulation and navigation tasks maintains a performance gap of less than 10% compared to policies trained purely on real-world data. These results confirm that GASE provides an efficient and highly effective solution for bridging the sim-to-real gap. Code will be released.
