How Good LLMs Are at Answering Bangla Medical Visual Questions? Dataset and Benchmarking

Abstract

Recent advancements in Large Language Models (LLMs) and Large Vision Language Models (LVLMs) have enabled general-purpose systems to demonstrate promising capabilities in complex reasoning tasks, including those in the medical domain. Medical Visual Question Answering (MedVQA) has particularly benefited from these developments. However, despite Bangla being one of the most widely spoken languages globally, there exists no established MedVQA benchmark for it. To address this gap, we introduce BanglaMedVQA, a dataset comprising clinically validated image-question-answer pairs, along with a comprehensive evaluation of current foundation models on this resource. Consistent with prior findings that report low performance of current models on English MedVQA benchmarks, our analysis reveals that Bangla performance is substantially lower, reflecting the challenges inherent to low-resource languages. Even top-performing models such as Gemini and GPT-4.1 mini fail to accurately answer specialized diagnostic questions, indicating severe limitations in fine-grained medical reasoning. Although certain open-source models, such as Gemma-3, occasionally outperform these models in general categories, they too struggle with clinically complex questions, underscoring the urgent need for top-notch evaluation method.

Does Language Shift Break Medical Vision-Language Models? Indonesian Radiology Visual Question Answering Case Study

Medical Vision-Language Models (VLMs) are typically evaluated on English radiology visual question answering benchmarks, leaving their robustness under non-English clinical language largely unexplored. We introduce IndoRad-VQA, an Indonesian adaptation of VQA-RAD, to assess whether medical VLMs retain radiology reasoning ability when questions are asked in Bahasa Indonesia. Radiology question-answer pairs are translated into Indonesian with self-evaluation-based quality control to preserve clinical meaning, terminology consistency, and answer equivalence. We evaluate general-purpose, Southeast Asian multilingual, and medical-specific VLMs under English and Indonesian prompting settings. Beyond accuracy, we quantify the language robustness gap between English and Indonesian inputs. We also conduct an error analysis to identify failure modes of question answering, such as yes/no flips, laterality errors, and output-language mismatches. Our findings show that strong performance on English medical VQA benchmarks does not necessarily translate to robust behavior in Indonesian clinical contexts. We observe a performance gap of 8 to 25 percent between the English and Indonesian settings, depending on the evaluation metric. These results highlight the need for more inclusive multilingual evaluation of medical multimodal foundation models. The dataset is available at https://huggingface.co/datasets/Lab-IS/IndoRad-VQA.

Polish Medical Visual Question Answering: Vision-Language Models Underutilize Visual Evidence

We introduce a Polish-language medical visual question answering (VQA) benchmark, built from Polish Board Certification Examination questions for licensed physicians and dentists pursuing specialist certification. The benchmark comprises image-containing questions spanning diverse medical specialties and visual domains, together with a text-only question answering (QA) control set. We evaluate Polish-oriented, general-purpose open-weight, and commercial vision-language models. The task remains challenging: the best model achieves 79.0% accuracy on the full VQA set, and only GPT-5.6 surpasses the approximate human reference on the subset with available candidate responses; all other evaluated models perform worse than humans. To assess visual grounding, we compare complete inputs with configurations omitting the image, the question, or both, and categorize questions by image importance. Models derive more useful information from the question text than from the image and perform worse on image-dominant questions. Across both QA and VQA, they nevertheless achieve above-chance accuracy from the answer choices alone, showing that non-trivial performance can persist even when key task components are missing.

A visual large language foundational model for medical image recognition using clinician-contributed online resources

Large language models (LLMs) have demonstrated strong capabilities across diverse domains, showing considerable potential in medicine. However, their application in medical settings remains limited by the scarcity of visual question answering (VQA) datasets that capture clinical reasoning and explicit image-text alignment. Here, we leverage de-identified medical images and expert commentaries shared on clinician-oriented social media. By combining an advanced LLM with clinician-in-the-loop verification, we established a rigorous pipeline to construct ThoughtMed-1M, a long-form medical VQA dataset containing over one million VQA pairs and designed to capture structured clinical logic and medical image-text alignment. To demonstrate its utility, we developed a FOundational LLM Trained on ThoughtMed-1M (FOLTMed). FOLTMed achieved state-of-the-art performance across 42 medical VQA benchmark datasets, with a macro accuracy of 85.4%, and generated more clinically coherent responses on the ThoughtMed-1M test set. It outperformed state-of-the-art models by 3--5% across factuality and similarity metrics, highlighting a scalable paradigm for advancing research on clinically grounded multimodal LLMs.
