First, do no harm: Breaking suicidogenic echo chambers in media recommendation

Abstract

Recommender systems generally optimises user engagement, but this approach is dangerous in mental health contexts. When vulnerable users show signs of suicidal ideation, standard algorithms often trap them in echo chambers of harmful content, worsening their psychological state. In response, we introduce RankAid, a re-ranking method that prioritises clinical safety alongside predictive relevance. It works as an add-on layer to existing models: it penalises risky items and boosts therapeutic content depending on the user's current level of vulnerability. We evaluated this approach using the MovieLens 1M dataset, where items were semantically annotated for clinical risk and therapeutic value using large language models. Our simulations show that our algorithm successfully blocks the recommendation of harmful content during crisis peaks, actively reshaping the feed to support emotional de-escalation. Furthermore, this safety intervention only causes a controlled, acceptable drop in standard accuracy metrics like NDCG. By using asymmetric hyperparameters, RankAid also gives system administrators the flexibility to tune the severity of the intervention based on specific clinical guidelines.

AI Chatbot Suicide Risk Detection and Response: Human Validation Study of the Open-Source VERA-MH Safety Evaluation

Millions of people now use generative AI chatbots for psychological support. Despite their promise, the most pressing question in AI for mental health is whether these tools are safe. The field currently lacks a validated, automated benchmark for evaluating AI chatbot safety, particularly for users at risk of suicide. The Validation of Ethical and Responsible AI in Mental Health (VERA-MH) evaluation was recently proposed to address this need. This human validation study examined the alignment of VERA-MH safety ratings with expert clinician judgments. We simulated conversations between large language model (LLM)-based users spanning a range of suicide risk levels and disclosure styles and general-purpose AI chatbots. Licensed mental health clinicians from Spring Health independently rated chatbot safety using the VERA-MH scoring rubric. An LLM-based evaluator ("judge") applied the same rubric to the same conversations. We examined agreement among clinicians, between clinician consensus and the LLM judge, and across different judge LLMs. Clinicians also rated user-agent realism, suicide risk, and disclosure. Clinicians showed strong agreement in safety ratings (chance-corrected inter-rater reliability [IRR] = 0.77), establishing a reliable clinical consensus reference. The LLM judge was strongly aligned with this consensus (IRR = 0.81), and ratings were stable across judge models and repeated evaluations. Ratings of user-agent realism and fidelity to intended suicide risk and disclosure styles were mixed. These findings support the reliability of VERA-MH as an open-source, fully automated benchmark for evaluating AI chatbot suicide risk detection and response. Because these results reflect an earlier version of the benchmark, future work should validate updated versions, assess generalizability and robustness, and expand VERA-MH to additional domains of AI safety in mental health.

LLM-Assisted Reranking to Operationalize Nuanced Objectives in Recommender Systems

Recommender systems have grown from content-organization tools into sophisticated systems that shape daily behavior. By controlling what we see, they shape what we perceive, raising concerns about filter bubbles, radicalization, polarization, and social inequality. Large language models (LLMs) enable more powerful personalization, intensifying these dynamics. Yet most recommenders are tuned for engagement or limited accuracy metrics, with little attention to broader social implications, e.g. how personalization reshapes exposure in socially consequential domains. We investigate whether LLM-assisted reranking, while improving personalization, inadvertently amplifies exposure to ideologically extreme or conspiratorial political content, a risk theorized but not empirically characterized in news recommendation. Using real news-consumption histories, we rerank YouTube's sidebar candidates through zero-shot, instruction-based prompting. We compare a baseline prompt with a constrained variant that preserves topical relevance and broadens ideological exposure while reducing conspiratorial or extreme content. Without constraints, reranking strengthened personalization but increased exposure to conspiratorial and extremist material for users whose histories contained such content. Lightweight prompt-level regularization reduced promotion of extreme content and increased ideological diversity, with modest relevance loss. Synthetic experiments suggest that LLMs rerank via statistical regularities in language rather than semantic understanding of ideology, clarifying why naive prompts amplify these patterns and why regularization can reshape them. Together, our results highlight the power of LLMs to operationalize contextual nuance in high-stakes recommendation, and the need to evaluate LLM-assisted personalization beyond accuracy and treat prompt design as a value-laden rather than neutral default.

Training Therapeutic Judges and Multi-Agent Systems for Human-Aligned Mental Health Support

Large language models show promise for mental health support, yet therapeutic quality improves only when evaluation functions as an actionable control signal rather than a passive metric. We introduce a framework that formulates therapeutic response generation as a decision-refinement problem driven by multi-dimensional, human-aligned evaluation. In Stage I, we introduce TheraJudge, an open-source therapeutic evaluator trained via preference-based optimization on human-annotated data to produce reliable judgments across 7 psychological dimensions. In Stage II, we introduce TheraAgent, which operationalizes TheraJudge's evaluations through a coordinated refinement process with specialized Critic, Coach, and Therapist roles that translate evaluative signals into targeted response revisions. Empirically, TheraJudge achieves strong agreement with clinician ratings, with intraclass correlation coefficients (ICC = 0.87-0.95), surpassing supervised baselines and strong closed-source judges, particularly on critical dimensions such as Safety, Relevance, and Empathy. Acting on these evaluations, TheraAgent yields a +0.43 improvement in human-rated therapeutic quality (on a 5-point scale) under blind evaluation, with 96% clinician inter-rater reliability. Low-quality responses (3\leq 3) improve by +2.45 points with a 94% recovery rate, demonstrating targeted correction of unsafe outputs. Overall, our results indicate that effective alignment of mental-health LLMs stems from acting on human-aligned evaluation, rather than relying solely on stronger generation.
