Abstract

Understanding tens-of-minutes surgical videos requires long-horizon temporal reasoning, answering what happens before, after, or across stages of a procedure by grounding the question in visual evidence spread across time. Existing approaches handle this poorly: a one-shot vision-language model (VLM) compresses the whole procedure to fit its context window and loses the detail a "before" or "after" question depends on, while video agents that train the model where to look are data-hungry and transfer poorly to out-of-domain surgery. We build an agent harness that separates reasoning from perception and improves by evolving context rather than optimizing weights. A text-only orchestrator plans which evidence to gather and issues an auditable sequence of tool calls, while frozen vision-language sub-agents execute each call over the pixels, viewing, cropping, inspecting frames, and retrieving external knowledge. We further propose a gradient-free, reward-gated Heuristic Skill Distillation loop that mines the agent's own low-scoring traces and keeps a candidate skill only when it raises a validation reward, yielding reusable retrieval skills, notably directed re-look. Growing an external skill library rather than tuning weights, the loop adapts from only about 100 labeled examples, far fewer than supervised or reinforcement fine-tuning requires. To evaluate this agent, we introduce MedClawBench, a de-leaked, doctor-grounded benchmark of 1,123 questions over self-built long neurosurgery recordings and a held-out public lecture-video test split. Across both datasets and all four evaluation dimensions, our agent consistently outperforms one-shot VLMs and general video-agent frameworks, with the largest gains on the long, out-of-domain neurosurgery videos. SurgLQA: Scalable Long-Horizon Surgical Video Question Answering

Surgical Video Question Answering (VideoQA) provides a promising paradigm for dynamic intraoperative interpretation, enabling real-time decision support and context-aware retrieval in clinical environments. Nevertheless, existing approaches are predominantly restricted to images or short clips, limiting their ability to model long-range procedural dynamics and causal dependencies across extended surgical workflows. To address this challenge, we propose SurgLQA, a unified long-horizon VideoQA framework for scalable surgical reasoning. This framework incorporates Faithful Temporal Consolidation (FTC), which leverages intrinsic temporal cues to construct compact long-range representations while preserving fine-grained temporal fidelity. Further, we develop Temporally-Grounded Multi-Policy Scaling (TMS), an adaptive test-time inference paradigm that strategically adjusts policy-level reasoning capacity within temporally grounded contexts. To facilitate systematic evaluation, we restructured a long-duration colonoscopy VideoQA benchmark, Colon-LQA, and conducted extensive experiments on Colon-LQA and REAL-Colon-VQA. Experimental results demonstrate that our approach achieves consistent performance gains in long-range reasoning with temporally grounded inference. Code link: https://github.com/RascalGdd/SurgLQA.

Online Video Agent Harness for Long Video Understanding

Long video understanding often behaves like a visual needle-in-a-haystack problem: query-relevant evidence is sparsely distributed across long temporal spans, while packing dense frames into a single VLM context incurs context rot and high cost. Existing video agents often rely on query-agnostic offline preprocessing or ad hoc tool sets, which can miss query-specific details and waste computation. In this work, we present VideoXAgent, a purely online video-agent harness for long video understanding that starts from the given video file and user query, plans and decomposes the task, invokes specialized expert tools on demand, and aggregates multimodal evidence to produce a final answer while resolving conflicts among observations. To support this on-demand invocation, we design a suite of heterogeneous expert tools guided by a data-driven taxonomy of atomic capabilities, spanning scripts, VLMs, and domain models (e.g., detection, OCR, ASR, face recognition). The harness further enforces objective evidence prompting and budget-aware control to curb hallucination and non-termination. Across Video-MME-Long, LongVideoBench-Long, LVBench, and MINERVA, VideoXAgent is competitive with frontier LMMs and video agents under a smaller context footprint---about 50k tokens of agent context per sample, even on hour-long videos. In particular, on complex video-reasoning benchmarks such as MINERVA, it matches this level while using only about 15% of the context of a 1,024-frame dense-packing baseline. Notably, the harness remains effective with a visually weak or even text-only orchestrator, suggesting that strong long-video understanding can emerge from progressive agentic evidence seeking rather than from packing the full video into a single context. LongVideo-R1: Smart Navigation for Low-cost Long Video Understanding

This paper addresses the critical and underexplored challenge of long video understanding with low computational budgets. We propose LongVideo-R1, an active, reasoning-equipped multimodal large language model (MLLM) agent designed for efficient video context navigation, avoiding the redundancy of exhaustive search. At the core of LongVideo-R1 lies a reasoning module that leverages high-level visual cues to infer the most informative video clip for subsequent processing. During inference, the agent initiates traversal from top-level visual summaries and iteratively refines its focus, immediately halting the exploration process upon acquiring sufficient knowledge to answer the query. To facilitate training, we first extract hierarchical video captions from CGBench, a video corpus with grounding annotations, and guide GPT-5 to generate 33K high-quality chain-of-thought-with-tool trajectories. The LongVideo-R1 agent is fine-tuned upon the Qwen-3-8B model through a two-stage paradigm: supervised fine-tuning (SFT) followed by reinforcement learning (RL), where RL employs a specifically designed reward function to maximize selective and efficient clip navigation. Experiments on multiple long video benchmarks validate the effectiveness of name, which enjoys superior tradeoff between QA accuracy and efficiency.
