Can Artificial Intelligence Support Healthcare and Mental Health Through Early Cyberbullying Detection ? The Impact of Emotion-Aware AI on Proactive Online Safety

Abstract

Healthcare systems, mental health, and public well-being are increasingly affected by cyberbullying and harmful online interactions. This paper presents CareGuard, an early-warning framework designed to support healthcare-driven mental health protection and proactive online safety through the detection of cyberbullying-related content using advanced natural language processing techniques. CareGuard integrates zero-shot semantic labeling with fine-tuned transformer-based models, including BERT, DistilBERT, and RoBERTa, to enable robust and context-aware classification across sensitive cyberbullying categories. To improve efficiency and reduce unnecessary computation in healthcare-oriented monitoring settings, the framework incorporates an emotion-aware filtering mechanism alongside cosine similarity-based semantic screening, allowing the system to focus on semantically relevant and emotionally salient content. Experimental results on benchmark datasets demonstrate that CareGuard effectively balances detection accuracy and computational efficiency, highlighting its potential for scalable deployment in healthcare systems, mental health monitoring, and online safety applications.

Improving Mental Health Screening and Early Risk Detection in Spanish

Early detection of mental health disorders is often limited by the lack of specialized resources in Spanish and the difficulty of analyzing long histories of social media posts. This paper addresses these challenges through three main contributions. First, we introduce three Spanish foundational models specifically adapted to the mental health domain through domain-specific pre-training. Second, we propose Incremental Context Expansion (ICE), an automatic relabeling methodology designed for early detection. ICE identifies the point at which cumulative messages provide enough evidence of a disorder, generating more informative training samples. Third, we provide a set of fine-tuned models using the samples generated with the ICE methodology for early risk detection tasks. Our results on three Spanish benchmarks show that combining these specialized models with ICE improves the state-of-the-art, reducing detection latency while maintaining high performance. All models are publicly available.

New AI-Driven Tools for Enhancing Campus Well-being: A Prevention and Intervention Approach

Campus well-being underpins academic success, yet many universities lack effective methods for monitoring satisfaction and detecting mental health risks. This dissertation addresses these gaps through prevention (improving feedback collection) and intervention (advancing mental health detection), unified under an integrated framework. For prevention, we developed TigerGPT, a personalized survey chatbot leveraging LLMs to engage users in context-aware conversations grounded in conversational design and engagement theory, achieving 75% usability and 81% satisfaction. To address its limitations in repetitiveness and response depth, we introduced AURA, a reinforcement-learning framework that adapts follow-up question types (validate, specify, reflect, probe) within a session using an LSDE quality signal (Length, Self-disclosure, Emotion, Specificity), initialized from 96 prior conversations. AURA achieved +0.12 mean quality gain (p=0.044, d=0.66), with 63% fewer specification prompts and 10x more validation behavior. For intervention, we examine Expressive Narrative Stories (ENS) for mental health screening, showing BERT(128) captures nuanced linguistic features without keyword cues, while conventional classifiers depend heavily on explicit mental health terms. We then developed PsychoGPT, an LLM built on DSM-5 and PHQ-8 guidelines that performs initial distress classification, symptom-level scoring, and reconciliation with external ratings for explainable assessment. To reduce hallucinations, we proposed Stacked Multi-Model Reasoning (SMMR), layering expert models where early layers handle localized subtasks and later layers reconcile findings, outperforming single-model solutions on DAIC-WOZ in accuracy, F1, and PHQ-8 scoring. Finally, a cohesive framework unifies these tools, enabling adaptive survey insights to flow directly into specialized mental health detection models.

Cyberbullying Governance on Social Media: A Unified Framework from Content Identification to Intervention

The proliferation of social media platforms and online communities has inadvertently catalyzed the spread of cyberbullying, hate speech, and other forms of online toxicity, making the effective governance of such harm a critical societal and computational challenge. While significant strides have been made in automating content moderation, existing research predominantly treats cyberbullying governance as passive, isolated detection at the post level. This reductionist view overlooks the continuous behavioral dynamics of users, the structural diffusion of toxic events, and the critical need for proactive mitigation. To bridge these gaps, this paper proposes a unified full-lifecycle governance framework that shifts the paradigm of cyberbullying governance from isolated static detection toward integrated, continuous, and proactive moderation. Drawing on cyberbullying research and adjacent fields, we systematically synthesize the state-of-the-art literature across four interconnected stages: (1) Content Identification, (2) User and Behavior Modeling, (3) Diffusion Dynamics and Early Warning, and (4) Intervention and Governance. Furthermore, we review available datasets and evaluation practices, and discuss emerging challenges including multimodality, explainability, algorithmic fairness, and the dual-use risks of generative AI, providing a roadmap for future research toward a safer and more resilient digital ecosystem.
