Abstract

Cervical vertebral maturation (CVM) assessment plays a pivotal role in orthodontic diagnosis and determining the optimal timing of treatment, especially for pediatric patients. In this paper, we propose LM-PCVMNet, a novel deep learning framework for automatic pediatric CVM staging. Specifically, our method integrates vertebral anatomical landmark information, heatmap-guided feature modulation, and metadata-informed similarity modeling into a unified learning framework. We introduce a heatmap-guided feature modulation module that enhances feature extraction by leveraging landmark-centered heatmaps to highlight morphologically relevant vertebral regions. A vertebral landmark-prompting block is designed to incorporate anatomical geometry into the representation learning process. Furthermore, we develop a learnable metadata supervised contrastive loss that adaptively modulates positive-pair similarity based on metadata similarity, enabling the model to learn more biologically consistent and discriminative features. To facilitate further research in pediatric orthodontic treatment, we additionally release PCVM+. It contains 1800 lateral cephalometric radiographs from real-world patients aged 3-15 years, with expert-annotated CVM stages, 13 vertebral anatomical landmarks, and corresponding metadata. We perform comprehensive experiments on two datasets, and the results show that our method achieves state-of-the-art performance, effectively improving landmark localization and classification accuracy over existing models. Code and dataset will be available at github.com/ybupengwang/LM-PCVMNet.

Automatic Cephalometric Landmark Localization on CBCT-Derived Digitally Reconstructed Radiographs for Skeletal Malocclusion Classification

Manual cephalometric landmark annotation is important for craniofacial assessment but is labor-intensive and difficult to scale. We introduce CephViT, a Vision Transformer-based model for automated 2D lateral cephalometric landmark localization, and evaluate its use in downstream skeletal malocclusion classification. CephViT was trained and benchmarked on a public lateral cephalogram dataset, achieving a mean radial error of 1.28 +/- 1.42 mm and a successful detection rate of 92.0% at 3.0 mm. Because the private evaluation cohort consisted of 3D CBCT scans, lateral cephalogram-like digitally reconstructed radiographs (DRRs) were generated from each volume and used as 2D inputs to the landmark localization model. Landmark coordinates were normalized into a common coordinate frame, and skeletal malocclusion classification was performed using landmarks shared between the reference and DRR-based pipelines. Classification performance using DRR-localized landmarks was comparable to that obtained using manually annotated reference landmarks, with accuracies of 70.0% and 68.3%, respectively. These results support the feasibility of automated cephalometric analysis on CBCT-derived DRRs for skeletal malocclusion assessment.

Progressive Deep Learning for Automated Spheno-Occipital Synchondrosis Maturation Assessment

Accurate assessment of spheno-occipital synchondrosis (SOS) maturation is a key indicator of craniofacial growth and a critical determinant for orthodontic and surgical timing. However, SOS staging from cone-beam CT (CBCT) relies on subtle, continuously evolving morphological cues, leading to high inter-observer variability and poor reproducibility, especially at transitional fusion stages. We frame SOS assessment as a fine-grained visual recognition problem and propose a progressive representation-learning framework that explicitly mirrors how expert clinicians reason about synchondral fusion: from coarse anatomical structure to increasingly subtle patterns of closure. Rather than training a full-capacity network end-to-end, we sequentially grow the model by activating deeper blocks over time, allowing early layers to first encode stable cranial base morphology before higher-level layers specialize in discriminating adjacent maturation stages. This yields a curriculum over network depth that aligns deep feature learning with the biological continuum of SOS fusion. Extensive experiments across convolutional and transformer-based architectures show that this expert-inspired training strategy produces more stable optimization and consistently higher accuracy than standard training, particularly for ambiguous intermediate stages. Importantly, these gains are achieved without changing network architectures or loss functions, demonstrating that training dynamics alone can substantially improve anatomical representation learning. The proposed framework establishes a principled link between expert dental intuition and deep visual representations, enabling robust, data-efficient SOS staging from CBCT and offering a general strategy for modeling other continuous biological processes in medical imaging.

Structure-aware Knowledge-guided Heterogeneous Mamba for Zygomaticomaxillary Suture Assessment

The Zygomaticomaxillary Suture is a key circummaxillary structure that connects the zygomatic bone and the maxilla, which serves as a primary site of resistance during maxillary advancement, and its maturation status directly influences the timing and efficacy of orthopedic interventions. However, accurate staging of ZMS maturation remains challenging due to subtle high-frequency transitions in suture lines and the global semantic ambiguity between adjacent stages. To address this, we present the first public ZMS dataset, comprising 3,790 ZMS images covering the entire age range from 4 to 24 years. Based on this dataset, we propose SKMamba, a Structure-aware and Knowledge-guided Mamba-based multi-modal framework for automated ZMS maturation assessment. SKMamba adopts a decoupled dual-path architecture that mimics the hierarchical diagnostic process used by experienced orthodontists. We first introduce an Implicit Edge Extractor (IEE), which leverages structural pre-training to reduce trabecular noise and accentuate sutural boundaries. Complementarily, a Cross-Modal Semantic Alignment (CSA) module is designed to incorporate anatomical descriptions from a large language model (LLM). This module helps align local morphological cues with global semantic descriptions while ensuring that objective morphological evidence remains the primary basis for decisions. Extensive experiments on our ZMS dataset demonstrate that SKMamba achieves state-of-the-art performance compared to existing methods. Code is available at https://github.com/galaxygxq1116/SKMamba.
