Abstract

The advent of Transformer and Mamba-based architectures has significantly advanced 3D medical image segmentation by enabling global contextual modeling, a capability traditionally limited in Convolutional Neural Networks (CNNs). However, state-of-the-art Transformer models often entail substantial computational complexity and parameter counts, which is particularly prohibitive for volumetric data and further exacerbated by the limited availability of annotated medical imaging datasets. To address these limitations, this work introduces SegMaFormer, a lightweight hybrid architecture that synergizes Mamba and Transformer modules within a hierarchical volumetric encoder for efficient long-range dependency modeling. The model strategically employs Mamba-based layers in early, high-resolution stages to reduce computational overhead while capturing essential spatial context, and reserves self-attention mechanisms for later, lower-resolution stages to refine feature representation. This design is augmented with generalized rotary position embeddings to enhance spatial awareness. Despite its compact structure, SegMaFormer achieves competitive performance on three public benchmarks (Synapse, BraTS, and ACDC), matching the Dice coefficient of significantly larger models. Empirically, our approach reduces parameters by up to 75x and substantially decreases FLOPs compared to current state-of-the-art models, establishing an efficient and high-performing solution for 3D medical image segmentation.

RefineFormer3D: Efficient 3D Medical Image Segmentation via Adaptive Multi-Scale Transformer with Cross Attention Fusion

Accurate and computationally efficient 3D medical image segmentation remains a critical challenge in clinical workflows. Transformer-based architectures often demonstrate superior global contextual modeling but at the expense of excessive parameter counts and memory demands, restricting their clinical deployment. We propose RefineFormer3D, a lightweight hierarchical transformer architecture that balances segmentation accuracy and computational efficiency for volumetric medical imaging. The architecture integrates three key components: (i) GhostConv3D-based patch embedding for efficient feature extraction with minimal redundancy, (ii) MixFFN3D module with low-rank projections and depthwise convolutions for parameter-efficient feature extraction, and (iii) a cross-attention fusion decoder enabling adaptive multi-scale skip connection integration. RefineFormer3D contains only 2.94M parameters, substantially fewer than contemporary transformer-based methods. Extensive experiments on ACDC and BraTS benchmarks demonstrate that RefineFormer3D achieves 93.44% and 85.9% average Dice scores respectively, outperforming or matching state-of-the-art methods while requiring significantly fewer parameters. Furthermore, the model achieves fast inference (8.35 ms per volume on GPU) with low memory requirements, supporting deployment in resource-constrained clinical environments. These results establish RefineFormer3D as an effective and scalable solution for practical 3D medical image segmentation.

BiSegMamba: Efficient Bidirectional Tri-Oriented Mamba for 3D Medical Image Segmentation

Accurate 3D medical image segmentation requires both long-range volumetric context and fine boundary preservation. CNN-based methods have limited global dependency modeling, while Transformer-based models are often computationally expensive for dense 3D inputs. Recent Mamba-based methods provide an efficient alternative, but existing volumetric designs still depend on repeated high-resolution scanning, forward-only sequential modeling, and fixed directional summation, causing high cost, scan-order bias, and suboptimal directional aggregation. We propose BiSegMamba, an efficient bidirectional tri-oriented Mamba network for 3D medical image segmentation. BiSegMamba follows a compact-to-detail design, where a progressive compacting stem (PCS) enables efficient latent-space reasoning while retaining shallow high-resolution features for reconstruction. A multi-scale spatial mixer (MSSM) captures local anatomical patterns in early stages, and the proposed bidirectional tri-oriented Ortho Mamba (Bi-ToOM) block models long-range dependencies from multiple orthogonal views using jointly processed forward and backward scan sequences. Adaptive directional fusion (ADF) learns input-dependent channel-wise weights across scan orientations, replacing fixed summation with orientation-aware fusion. Experiments on a collected carotid CTA dataset and three public benchmarks, BraTS2023, ACDC, and AMOS-CT, show that BiSegMamba generalizes well across vascular, cardiac, brain tumor, and abdominal multi-organ segmentation tasks. Compared with SegMamba-V2, BiSegMamba achieves slightly better performance on BraTS2023 and clear improvements on ACDC and the carotid dataset, while reducing computational cost by up to 77.9% FLOPs, demonstrating a strong accuracy-efficiency balance for general 3D medical image segmentation.

LightMedSeg: Lightweight 3D Medical Image Segmentation with Learned Spatial Anchors

Accurate and efficient 3D medical image segmentation is essential for clinical AI, where models must remain reliable under stringent memory, latency, and data availability constraints. Transformer-based methods achieve strong accuracy but suffer from excessive parameters, high FLOPs, and limited generalization. We propose LightMedSeg, a modular UNet-style segmentation architecture that integrates anatomical priors with adaptive context modeling. Anchor-conditioned FiLM modulation enables anatomy-aware feature calibration, while a local structural prior module and texture-aware routing dynamically allocate representational capacity to boundary-rich regions. Computational redundancy is minimized through ghost and depthwise convolutions, and multi-scale features are adaptively fused via a learned skip router with anchor-relative spatial position bias. Despite requiring only 0.48M parameters and 14.64~GFLOPs, LightMedSeg achieves segmentation accuracy within a few Dice points of heavy transformer baselines. Therefore, LightMedSeg is a deployable and data-efficient solution for 3D medical image segmentation. Code will be released publicly upon acceptance.
