Beyond English benchmarks: clinical llm evaluation in Brazilian Portuguese

Abstract

Large Language Models are transforming the support for clinical decision and their application in real scenarios. Yet, most benchmarks are conducted in English, and cross-lingual evaluation is needed to tackle the language gaps in global access. We introduce ClinicalBr, the first bilingual benchmark for clinical decision built from real Brazilian case reports. The corpus contains 2,892 cases drawn from 28 SciELO medical journals, spanning 18 specialties, and is structured as parallel Portuguese-English pairs. Each case supports four evaluation tasks: diagnosis retrieval, differential diagnosis, exam recommendation, and treatment planning. We evaluate four models: MedGemma-27B, Sabiá-4, DeepSeek-R1, and o3-mini, across both languages. The central finding is that the Portuguese-English performance gap is task-dependent, not general. In diagnosis retrieval, English yields a consistent advantage across all models, with +7.5-12.1 accuracy points. This advantage disappears in differential diagnosis, exam recommendation, and treatment planning, where confidence intervals cross zero for most models and Portuguese completeness scores are marginally higher. Brazilian-endemic conditions proved easier than the full corpus, not harder, indicating that tropical presentations are adequately represented in current pre-training. Exam recommendation was the hardest task across all models and both languages, with F1 scores below 0.10, well below the differential diagnosis ceiling of 0.20-0.27.

Teaching LLMs Brazilian Healthcare: Injecting Knowledge from Official Clinical Guidelines

Brazil's Unified Health System (SUS) relies on official clinical guidelines that define diagnostic criteria, treatments, dosages, and monitoring procedures for over 200 million citizens. Yet current LLMs perform poorly on this guideline-specific knowledge, and no benchmark evaluates clinical recall grounded in Brazilian Portuguese protocols. We address this gap by adapting Qwen2.5-14B-Instruct to the Brazilian clinical domain. From 178 official guidelines (~5.4M tokens), we generate ~70M tokens of synthetic data in three formats -- rephrases, wiki-style articles, and question-answer pairs -- using four generator LLMs. We then apply continual pre-training followed by Group Relative Policy Optimization (GRPO). We introduce HealthBench-BR, with 1,780 balanced true/false clinical assertions, and PCDT-QA, with 890 open-ended clinical questions scored by an LLM judge. Our best model achieves 83.9% on HealthBench-BR and 85.4% on PCDT-QA, outperforming GPT-5.2, Claude Sonnet 4.6, Gemini 3.1 Pro, and Google AI Overview's web-grounded RAG despite having only 14B parameters. Ablations show that generator diversity and reinforcement learning are critical to these gains. We release all datasets, benchmarks, and model weights to support reproducible clinical NLP research for Brazilian Portuguese. Prompting language influences diagnostic reasoning and accuracy of large language models

Large language models (LLMs) are increasingly explored for clinical decision support, yet most evaluations are conducted in English, leaving their reliability in other languages uncertain. Here we evaluate the impact of prompting language on diagnostic reasoning and final diagnosis accuracy by comparing English and French performance across five LLMs (o3, DeepSeek-R1, GPT-4-Turbo, Llama-3.1-405B-Instruct, and BioMistral-7B). A total of 180 clinical vignettes covering 16 medical specialties were assessed by two physicians using an 18-point scale evaluating both diagnosis accuracy and reasoning quality. Four of the five models performed better in English (mean difference 0.37-0.91, adjusted p < 0.05), with the gap spanning multiple aspects of reasoning, including differential diagnosis, logical structure, and internal validity. o3 was the only model showing no overall language effect. These findings demonstrate that prompting language remains a critical determinant of LLM clinical performance, with implications for equitable linguistico-cultural deployment worldwide.

Reassessing High-Performing LLMs on Polish Medical Exams: True Competence or Bias-Driven Performance?

Large language models (LLMs) in medicine are mainly evaluated using multiple-choice question answering (MCQA), which can overestimate real clinical ability due to guessing strategies and answer biases. To address these limitations, we introduce an expanded and more challenging benchmark based on Polish medical exams, adding over 15,000 questions, two new domains, and four structural modifications that reduce MCQA-specific artifacts and better test reasoning. We evaluate 21 LLMs and show that evaluation design strongly affects results. Under our harder setup, the best model (Qwen3.5-122B) drops by 28.4 and 31 pp on English and Polish exams, respectively. Despite low evidence of data contamination, standard MCQA scores do not reliably reflect true medical competence. To facilitate further research, we make our benchmark publicly available.
