Evidence-Bounded Mental Health Reasoning from Heterogeneous Speech Protocols

Abstract

Computational mental health screening using multimodal speech and text has shown great promise. However, existing models often assume all clinical speech protocols carry equivalent evidentiary validity. In reality, heterogeneous protocols, from free interviews to fixed reading tasks, support fundamentally different evidence. Forcing uniform reasoning flattens these boundaries, causing models to hallucinate symptoms from irrelevant text or overclaim support. Even advanced long chain-of-thought LLMs fail to resolve this issue, as free-form reasoning can exacerbate boundary violations. To address this, we reformulate multimodal screening as an evidence-bounded reasoning problem. We introduce the Evidence Package Benchmark, integrating 1,870 packages across six heterogeneous sources with explicit modality masks and evidence permissions. We further propose EviBound, a protocol-aware evidence control framework. Unlike direct LLM prompting, EviBound uses a profile-aware planner to restrict reasoning scope, orchestrates evidence tools via five-way acoustic consensus, and enforces a boundary critic to suppress unsupported claims. Empirical results show EviBound achieves a held-out test Depression AUROC of 0.8658, exceeding the strongest direct omni-modal baseline by +0.0811 AUROC while maintaining zero claim violations. Our work moves beyond unconstrained accuracy toward evidence-consistent, protocol-aware systems for safer clinical NLP research.

A Multi-Agent Audit Framework for High-Stakes Reasoning: Evaluation and Interpretability in Clinical Mental Health Screening

High-stakes reasoning tasks necessitate transparent and verifiable workflows, yet conventional single-model large language models (LLMs) often struggle with hallucination and low interpretability under zero-shot paradigms. To address this general AI challenge, we propose a Multi-Agent Audit Framework that simulates a collaborative, multi-step verification process. We empirically validate this architecture in the sensitive domain of clinical mental health screening using a modular LangChain workflow. Our framework decomposes the reasoning process into a Perception Agent, Knowledge Retrieval-Augmented Generation (RAG), Chain-of-Thought (CoT) clinical inference, and a critical Audit verification stage. We evaluated this framework on the DAIC-WOZ dataset using locally deployed open-source models. Experimental results demonstrate that our multi-agent pipeline significantly outperforms single-agent baselines, reducing the Mean Absolute Error (MAE) for PHQ-8 depression severity prediction from 5.35 to 5.02. By exposing cross-agent validation traces, the framework mitigates reasoning drift and provides highly interpretable diagnostic rationales, offering a generalizable paradigm for reliable AI-assisted decision support beyond isolated model scaling. When Symptoms Are Not Enough: Evidence-Weighting Patterns in Large Language Model Psychiatric Screening

As demand for mental health care outpaces clinician-delivered assessment, scalable screening tools are increasingly needed. Large language models (LLMs) may identify psychiatric risk from patient narratives, but their reliability across diagnoses, demographic subgroups, and evidence-use patterns remains uncertain. We introduce a SCID-anchored benchmark of 555 semi-structured experiential interviews paired with diagnostic reference labels for anxiety disorder, major depressive disorder, post-traumatic stress disorder, and any current mental health disorder. Using zero-shot task-specific prompting, we evaluated five state-of-the-art LLMs and examined whether false-negative errors reflected missed psychiatric evidence or differential weighting of symptom, functional-impairment, and protective-context cues. Performance varied across tasks and models, with accuracy ranging from 0.49 to 0.86 and Matthews correlation coefficients from 0.16 to 0.38. GPT-4.1 Mini and GPT-5 Mini showed the most consistent disorder-specific accuracy. Subgroup analyses found higher depression-classification accuracy among male than female participants, no consistent age-related pattern, and modest non-uniform variation across race strata. Evidence-integration analyses showed that false-negative anxiety and PTSD classifications often contained explicit symptom evidence but were accompanied by preserved functioning, coping ability, or social support. Functional-impairment evidence shifted model outputs toward positive classifications, whereas protective-context evidence shifted outputs away. These findings suggest that LLMs may support scalable psychiatric screening, but their tendency to discount symptom evidence in the presence of preserved functioning or protective context requires careful validation before clinical deployment.

CPEMH: An Agentic Framework for Prompt-Driven Behavior Evaluation and Assurance in Foundation-Model Systems for Mental Health Screening

This paper presents CPEMH, an agentic framework designed to evaluate prompt-driven behavior in foundation-model systems operating on transcript-based datasets for mental-health screening. CPEMH serves as an engineering methodology for behavioral assurance in large-scale language systems, introducing an orchestrated architecture that autonomously performs the design, evaluation, and selection of prompt strategies, enabling systematic control of behavioral variability across contexts. Its modular agentic design, combining orchestrator, inference, and evaluation agents, ensures traceability, reproducibility, and robustness throughout the prompting lifecycle. A case study on automated depression screening from interview transcripts demonstrates the framework's capacity to stabilize and audit foundation-model behavior in conversational and clinically sensitive domains. Lessons learned emphasize the role of modular orchestration in behavioral assurance, the prioritization of stability over architectural complexity, and the integration of F1, bias, and robustness as core acceptance criteria.
