NeoRed: A Knowledge-Logic-Alignment Multimodal Large Language Model for Neonatal Respiratory Disease Diagnosis

Abstract

Neonatal respiratory diseases are a major cause of neonatal morbidity and mortality, posing substantial challenges in clinical practice. Despite recent advances, existing Multimodal Large Language Models (MLLMs) face two key limitations in neonatal diagnosis: (1) domain gap arising from predominantly adult training data; (2) insufficient integration of multidimensional clinical context for accurate diagnosis. To address these challenges, we collect two real-world clinical datasets (NeoCXR and NeoCXR-EV) and propose NeoRed, to the best of our knowledge, the first MLLM tailored for neonatal respiratory disease, filling the gap in neonatal diagnostic reports generation. To enhance joint diagnosis from heterogeneous clinical context and chest X-rays, we design a novel Knowledge-Logic-Alignment (KLA) framework which constrains model behavior from three perspectives: 1) Knowledge Prior Injection (KPI) incorporates neonatologist-inspired diagnostic priors into multimodal representations, guiding disease-specific attention across modalities; 2) Diagnostic Logic Constraint (DLC) aligns the semantics of generated reports with multimodal diagnostic logic; and 3) Visual Semantic Alignment (VSA) establishes semantic correspondence between visual features and imaging conclusions. Extensive experiments demonstrate that NeoRed enables accurate neonatal diagnostic reports generation, achieving ROUGE-L of 53.29% and Clinical Efficacy F1 score of 65.19% on NeoCXR, outperforming existing MLLMs. NeoRed also preserves competitive report generation performance on adult benchmarks (MIMIC-CXR and IU-Xray). Datasets will be available upon application.

Multimodal Large Language Model driven Radiology Report Generation with Clinical Knowledge Enhancement

Radiology report generation (RRG) has attracted significant attention due to its potential to reduce the workload of radiologists. The performance of current RRG approaches remains unsatisfactory against clinical standards. This paper introduces a novel RRG method, MLLM-RRG, that integrates multimodal large language models (MLLMs) with various types of clinical knowledge to generate accurate and comprehensive chest X-ray reports. Our method first designs a referring anatomical feature extractor that leverages anatomical knowledge to analyze different regions of the chest X-ray image and extract visual features without explicitly detecting regions. Next, based on the MLLM's decoder, we develop a multimodal report generator that leverages multimodal prompts constructed from dedicated visual features and textual instructions to produce the radiology report in an auto-regressive way. Finally, we introduce a disease-oriented clinical classification and alignment scheme in a multi-task learning manner to leverage disease knowledge to better preserve the clinical relevance among the generated reports. Once the model is trained, we also introduce a novel clinical quality reinforcement learning strategy to enhance the MLLM with report knowledge, further refining the tones of the generated reports towards radiologists. Extensive experiments on the MIMIC-CXR and IU X-Ray datasets demonstrate the superiority of our method over the state of the art. RespiraMFM: A Multimodal Foundation Model with Contrastive Audio-Language Alignment for Respiratory Disease Identification

Respiratory diseases remain a leading cause of global mortality, where timely and accurate diagnosis is critical to improving patient outcomes and reducing healthcare burdens. While prior work has explored audio-based models for respiratory disease detection, such unimodal approaches often suffer from limited generalizability and diagnostic precision. In this paper, we propose RespiraMFM, a Multimodal Foundation Model that integrates respiratory sounds with patient medical history and symptoms to enhance diagnostic accuracy and disease detection capabilities. We introduce an effective contrastive alignment strategy for audio-text multimodal integration, allowing the model to learn better cross-modal representations between respiratory sounds and corresponding textual clinical information. We evaluate RespiraMFM across five major respiratory diseases using seven real-world datasets in both supervised fine-tuning and zero-shot settings, achieving a 9.15% improvement in AUROC on supervised tasks and a 20.98% gain on zero-shot tasks over existing baselines. These findings underscore the potential of our framework to advance early diagnosis and improve clinical decision-making in respiratory disease management.

Lung-R1: A Knowledge Graph-Guided LLM for Pulmonary Diagnostic Reasoning

Diagnosing pulmonary diseases requires integrating heterogeneous evidence amid phenotypic variability and cross-disease overlap. Although large language models (LLMs) have shown progress on pulmonary knowledge question answering (QA) and information-processing tasks, reliable pulmonary diagnosis requires patient-specific, relation-aware reasoning over electronic medical record (EMR) evidence rather than isolated knowledge recall. We define this gap between pulmonary knowledge and case-level diagnostic reasoning as the Pulmonary Knowledge-to-Diagnosis Gap. To address it, we introduce LungKG, the first structured pulmonary knowledge graph for diagnostic knowledge organization and record-grounded reasoning. LungKG contains 59,038 nodes and 164,308 edges across 15 entity types and 112 relation types, serving as both a reusable pulmonary knowledge resource and the foundation for LungKG-guided model adaptation. Built on LungKG, we propose Lung-R1, a LungKG-guided pulmonary LLM trained through KG-constrained reasoning-chain construction and KG-guided reinforcement learning. In a 20-system evaluation, Lung-R1-14B achieves state-of-the-art performance across Choice, Pulmonary-QA, and EMR Diagnosis, reaching an EMR Diagnosis score of 4.3583 and surpassing the strongest non-Lung-R1 baseline by 0.1476 points. These results demonstrate the value of LungKG-guided training for EMR-based pulmonary diagnosis.
