Co-STAR: Cognitive Stimulation Therapy by an Autonomous Robot for Dementia -- A One-Week In-Home Study

Abstract

Cognitive therapies have been shown to enhance the quality of life and well-being of people living with dementia (PwDs). However, their use remains limited due to a shortage of trained professionals and the significant time and training required of informal caregivers. To address this gap, we developed and deployed a social robot capable of autonomously delivering cognitive stimulation therapy (CST) in the home. Nine PwDs participated in a one-week (77 days) study that involved daily robot-led sessions. Participants engaged positively with the system, completing nearly half of the scheduled sessions, an adherence rate higher than typically observed in caregiver-led CST. Our findings also highlight the crucial role of family members, who often supported session initiation and occasionally joined the activities, enriching the interactions. This work demonstrates the feasibility and potential of socially assistive robots to deliver in-home cognitive therapy, offering a scalable approach to extend access to dementia care.

Factors Influencing Conversational Engagement in Robot-Delivered Individual Cognitive Stimulation Therapy (iCST) for Dementia in Home Settings

Social robots offer a promising means of supporting cognitive therapies for dementia care by guiding structured conversation and therapeutic activities. However, little is known about the conversational dynamics that emerge during robot-delivered cognitive stimulation therapy (CST) sessions. This study analysed the interaction patterns from robot-delivered individual CST (iCST) sessions conducted with people living with dementia in home settings. Our Co-STAR (Cognitive Stimulation Therapy by an Autonomous Robot) system was deployed in the homes of eight PwDs for one week, who completed 30-minute sessions. Conversational metrics, including words per turn, speech production rate, response duration, response latency, and self-referential language, were analysed to examine how conversational engagement is shaped by prompt personalisation, interaction phase, and participant characteristics. The findings highlight three key interactional properties of robot-delivered iCST. First, personalised prompts significantly increase response duration, self-referential language, and overall engagement compared to generic prompts. Second, conversational behaviour changes within sessions, with a reduction in the verbal output and autobiographical engagement observed during later interaction phases, which suggests cognitive fatigue. Third, first-session conversational metrics were associated with long-term participation, while living situation influenced conversational engagement patterns. These findings provide empirical insights into the factors that shape conversational engagement in robot-delivered iCST. They inform the design of adaptive conversational robots for dementia therapy.

A Deployable Architecture for Robot-Mediated Tasks (DART): Evaluation in Socially Assistive Robot-Guided Cognitive Behavioral Therapy Exercises

Socially assistive robots (SARs) can support structured health and well-being interventions, but hardware and cost constraints limit interaction complexity and longitudinal real-world deployments. We present DART: Deployable Architecture for Robot-Mediated Tasks, an architecture that extends SARs through a web application and cloud infrastructure, enabling visual content, user input, remote computation, and persistent data storage synergistically with the robot's physical embodiment, speech, and movement. We evaluated DART by instantiating it in an interatively-developed full-stack HRI system for helping university students with elevated generalized anxiety to complete cognitive behavioral therapy (CBT) homework exercises. The resulting system, which used the low-cost open-source Blossom robot platform, was refined and evaluated through a participatory design process and multiple user studies, and finally evaluated in an in-lab study with 103 participants, and then a six-week in-home deployment with four participants. In the in-lab evaluation, participants showed significant within-session reductions in stress, state anxiety, and negative affect, and gave the platform a mean System Usability Scale score of 78.89. In the home deployment, the mean System Usability Scale score was 87.5, with positive qualitative feedback on usability. Participants across both groups identified speech input, visual presentation, and web-robot synchronization as priorities for improvement. These findings validate DART as an effective architecture for extending the capabilities of a low-cost SAR in both in-lab single-session and in real-world longitudinal deployments.

Perception of Social Robots as Communication Partners in Healthcare for Older Adults

Addressing the global caregiver shortage through socially assistive robots necessitates a deep understanding of their psychological and physiological impacts on older adults during human-robot interaction (HRI). This study addresses whether social robots can serve as effective interaction partners compared to humans, and if "positive prompts" can similarly enhance these interactions. We conducted a comparative study with 35 participants (aged 70+). Our multi-modal analysis, integrating facial expression data, heart rate variability, and subjective questionnaires, revealed no significant differences in overall stress levels between human and robot interactions. Facial expression analysis confirmed that the robot was accepted as a valid interaction partner, while physiological data showed slightly lower heart rates during robot interactions, suggesting a more relaxed state compared to human-led sessions. These findings indicate that social robots can engage older adults without inducing psychological strain and are capable of alleviating caregiver burden by performing structured tasks, such as health-sensing surveys. Future work should address the identified "appearance-content mismatch" in robot design to facilitate even more natural and effective interactions.
