Beyond wheelchairs and blindfolds: Investigating disability stereotypes in T2I models with INCLUDE-BENCH

Abstract

Text-to-image (T2I) models have been shown to exhibit social biases. Prior work has mainly focused on gender, skin tone, and cultural representation within restricted occupational associations, and emerging benchmarks increasingly incorporate these dimensions. However, disability remains systematically underexplored. Current evaluation practices often fail to align with sociologically grounded definitions of stereotyping, limiting principled assessment of representational harms toward people with disabilities (PWD). To address this, we introduce INCLUDE-BENCH, the first large-scale benchmark for evaluating disability-related bias in T2I models. INCLUDE-BENCH comprises 119K generated images based on prompt design across multiple bias dimensions and both static and dynamic contexts. We evaluate 15 open-source and 2 closed-source models. Our key findings reveal that: (1) mobility-impaired and default disability prompts predominantly yield wheelchair depictions across all models; (2) disability-conditioned generations consistently exhibit less diversity; (3) stereotypical portrayals demonstrate stronger disability-text alignment; and (4) we introduce the Stereotype Content Model (SCM) Score, demonstrating that T2I models reflect real-world stereotypical associations.

Context Matters: Auditing Gender Bias in T2I Generation through Risk-Tiered Use-Case Profiles

Text-to-image (T2I) generative models are increasingly used to produce content for education, media, and public-facing communication, and are starting to be integrated into higher-impact pipelines. Since generated images tend to reinforce stereotypes, producing representational erasure via "default" depictions and shaping perceptions of who belongs in certain roles, a growing body of work has proposed metrics to quantify gender bias in T2I outputs. Yet existing evaluations remain fragmented. Metrics are often reported without a shared view of what they measure, what assumptions they entail, or how their results should be interpreted under different deployment contexts. This limits the usefulness of gender bias measurement for both technical auditing and emerging governance discussions. We propose a risk-aligned auditing framework for gender bias in T2I models composed of three constituents that connects risk categories, evaluation metrics, and harms. First, we identify risk-tiered use-case profiles aligned with the EU AI Act's risk categories to motivate why auditing expectations may vary with deployment contexts and stakeholder exposure. Second, we construct a metric catalog that consolidates gender-bias evaluation methods and organizes them in three measurement categories: gender prediction, embedding similarity, and downstream task. Third, we introduce a harm typology that maps context-dependent harm categories (e.g., representational, quality-of-service) to specific risk-tired scenarios. Finally, we introduce THUMB cards (Text-to-image Harms-informed Use-case-aligned Metrics of gender Bias) that help formulate auditing systematically by the incorporation of context, scenario and bias manifestation, harm hypotheses, and audit strategy.

Harm is not Universal: Community-Specific Toxicity Detection is Urgently Needed

State-of-the-art toxicity detectors for text-to-image generation adopt a one-size-fits-all approach: a single universal model applying fixed safety guidelines to all users. Our empirical evidence shows that these detectors fail to shield marginalized communities: approximately 35% of generated images labeled safe are considered harmful by disability communities. In this position paper, we argue for community-specific toxicity detection (CTD). To demonstrate its feasibility, we collaborate with disability experts to develop safety guidelines for two communities: dwarfism and blind/low vision. Using a dataset of 2,400 annotated T2I-generated images we demonstrate that both large vision-language models and existing general-purpose toxicity detectors catastrophically fail to recognize harmful content under these guidelines in zero-shot settings with F1 score lower than random guessing (F1 0.32 and 0.37). Promisingly, prompt-based adaptation methods (ICL, VQA) substantially improve harm detection performance (GPT-4o: F1 0.50 and 0.78), while parameter-efficient fine-tuning improves smaller models (0.5b-7b with best F1 0.48 and 0.59) with less than 100 demonstrations, but remains sensitive to evolving guidelines. Despite these gains, CTD performance remains far below F1 0.9\approx 0.9 achieved for general-purpose toxicity detection, highlighting the challenge and the need for sustained research effort.

Investigating Social Bias in Narrative Image Generation

Text-to-image (T2I) generation models are increasingly embedded in applications such as media content creation and education, raising concerns about how their outputs may reproduce social biases. Prior work has shown that T2I models exhibit social biases, yet existing evaluations largely focus on a photo generation task. As a result, it remains unclear whether and how such biases manifest in more narrative visual formats, such as storyboards and comics, where characters and events are presented across multiple panels. In this work, we compare bias expression across photo, storyboard, and comic generation in six T2I models by adapting BBG, a text-based bias evaluation framework, to image generation. Our results show that proprietary models generate 25.9% biased outputs in photo generation on average, with biased outputs increasing by 9.6pp in storyboard generation and 18.2pp in comic generation. We also find that photos mainly encode biases through subtle visual cues, while storyboards and comics reveal them more explicitly through event sequencing, character positioning, narrative resolution, and textual elements. These findings show that biases that remain less visible in photo generation may surface in narrative visual formats, highlighting the importance of evaluating T2I systems with diverse visual formats beyond photo generation.
