Feature-Space Guided Diffusion for Realistic Ultrasound Image Synthesis

Abstract

Conditional diffusion models can generate anatomically plausible medical ultrasound (US) images, but anatomical plausibility alone does not ensure realistic B-mode appearance. Most US pipelines adapt standard generative architectures and condition them on anatomical masks, or use guidance mechanisms that reinforce the same anatomical signal. However, B-mode US images are shaped by acquisition-dependent properties such as speckle texture, tissue contrast, and attenuation. Using a frozen US foundation model, we show that standard conditional diffusion baselines remain separated from real images in representation space. In this work, we propose Feature-Space Candidate Guidance (FSCG), a training-free sampling strategy to reduce this gap. At sampling time, FSCG applies local k-NN feature correction and selects the best of multiple stochastic candidates according to their feature-space energy. In this way, the mask defines the anatomy, while FSCG steers samples toward the real US domain. Across three different datasets, FSCG reduces average FID64 by 56%, FID192 by 57%, and nearest-neighbour feature distance by 47% over standard conditional diffusion sampling, outperforming alternative inference-time guidance baselines. The results suggest that domain-aware feature representations can reveal and reduce realism gaps in medical diffusion synthesis without retraining the generator. A Foundational EDM2-Based Generative Model for High-Resolution Synthetic Fetal Ultrasound Imaging from Open Datasets

Prenatal ultrasound imaging is key for assessing fetal health, but AI progress is limited by scarce, privacy-restricted, and hard-to-annotate datasets. We propose a high-resolution fetal ultrasound synthesis framework based on the EDM2 diffusion architecture, trained on multiple public datasets to generate 512x512 images across six anatomical classes. Our method achieved improved image quality with lower FID scores and enhanced downstream fetal plane classification, reaching 93.36% ensemble accuracy after fine-tuning, surpassing real-data-only training. Clinical evaluation by an experienced fetal ultrasound specialist (10+ years) on 100 images yielded a mean realism score of 2.67/5, with real images rated higher than synthetic. Artefacts included smoothing, speckle irregularities, and anatomical inconsistencies. Multi-Channel Uncertainty-Weighted Score Matching for Conditional Diffusion in Medical UDA

Robust medical image segmentation across modalities remains challenging due to severe domain shifts and the lack of target-domain labels. While diffusion models have been explored for cross-domain generation and augmentation, target-domain conditional diffusion training typically relies on highly noisy pseudo masks; naively conditioning on a single Arg-Max pseudo-label can corrupt diffusion training and downstream segmentation. We propose UPDiff-UDA, a unified UDA framework whose core is an uncertainty-guided training objective for target-domain conditional diffusion. Given an imperfect source-trained segmenter, we use its per-pixel softmax distribution to form ranked pseudo-label maps (Arg-Max, Arg-2nd, Arg-3rd, ...). Each map yields a conditional score estimate, and we aggregate them via pixel-wise confidence weighting to obtain an uncertainty-reweighted score for score matching, improving robustness to pseudo-label noise while leveraging alternative plausible labels in uncertain regions. We further provide a theoretical justification showing that confidence-weighted aggregation follows a minimum-MSE convex-combination principle under the segmenter-induced surrogate label distribution. To improve pseudo-condition quality, we also introduce a feature-guided, low-degree-of-freedom Bézier curve adaptation to reduce appearance gaps. Experiments on multiple public datasets and modality shifts show that UPDiff-UDA generates high-fidelity labeled target-style samples for augmentation and consistently outperforms strong UDA baselines. Null-Space Diffusion Restoration with Adaptive Uncertainty-Guided Fusion for Ultrasound Speckle Reduction

Ultrasound B-mode imaging commonly suffers from speckle noise and artifacts, requiring a delicate balance between contrast, resolution, and preservation of anatomical structures. Although recently developed despeckling methods have achieved some progress, supervised learning approaches remain fundamentally limited by the ground truth paradox, which arises from the absence of noise-free, ground truth reference images in in vivo scenarios. Existing unsupervised diffusion-based methods typically enforce data consistency directly in the nonlinear log-compressed domain, which can disproportionately amplify background artifacts when mapped back to the envelope domain. To overcome these limitations, we propose an uncertainty-guided null-space diffusion (UGNS) framework, a novel label-free solution that enforces consistency correction on a stabilized positive-envelope proxy obtained via inverse log compression. The proposed UGNS introduces several technical novelties: (a) extraction of a structural prior in the stabilized envelope domain to produce a robust signal envelope that preserves anatomical structure, (b) development of an adaptive range-null reconstruction mechanism that uses an adaptive weight mask to preserve tissue regions via range-space projection, and (c) introduction of uncertainty-guided fusion in an adaptive way to mitigate sampling variability. Extensive and comparative experiments were conducted using the PICMUS benchmark and in vivo datasets. The results demonstrate that UGNS achieves competitive generalized contrast-to-noise ratio (gCNR) values across diverse datasets. In addition, it is successfully validated that UGNS effectively suppresses speckle noise while preserving fine spatial resolution. Code is available at https://github.com/yousirong/UGNS.git.
