SocialWise: LLM-Agentic Conversation Therapy for Individuals with Autism Spectrum Disorder to Enhance Communication Skills

Authors: Albert Tang

Abstract

Autism Spectrum Disorder (ASD) affects more than 75 million people worldwide. However, scalable support for practicing everyday conversation is scarce: Low-cost activities such as story reading yield limited improvement. At the same time, effective role-play therapy demands expensive, in-person sessions with specialists. SocialWise bridges this gap through a browser-based application that pairs LLM conversational agents with a therapeutic retrieval augmented generation (RAG) knowledge base. Users select a scenario (e.g., ordering food, joining a group), interact by text or voice, and receive instant, structured feedback on tone, engagement, and alternative phrasing. The SocialWise prototype, implemented with Streamlit, LangChain, and ChromaDB, runs on any computer with internet access, and demonstrates how recent advances in LLM can provide evidence-based, on-demand communication coaching for individuals with ASD.

From Synthesis to Clinical Assistance: A Strategy-Aware Agent Framework for Autism Intervention based on Real Clinical Dataset

The development of AI-assisted Early Intensive Behavioral Intervention (EIBI) for Autism Spectrum Disorder (ASD) is severely constrained by data scarcity. Furthermore, while Applied Behavior Analysis (ABA) serves as the gold standard for clinical intervention, general-purpose Large Language Models (LLMs) struggle to strictly adhere to its standardized procedures, often resulting in interactions that are linguistically fluent but strategically inconsistent. To address these challenges, we introduce ASDAgent, a strategy-aware framework designed to unify high-fidelity intervention dialogue synthesis and clinical decision support. ASDAgent incorporates two specialized components to solve distinct problems: (i) a DoctorAgent equipped with an Observe-Think-Act-Correct (O-T-A-C) reasoning loop, which resolves the issue of strategy collapse in LLMs by making ABA execution explicit and controllable; and (ii) a ChildAgent that utilizes probabilistic behavior modeling to mitigate data homogeneity, simulating diverse and non-deterministic ASD response patterns. Experiments demonstrate that dialogues generated by ASDAgent closely mirror the strategy distribution of human therapists (KL divergence: 0.083). In real autism intervention, ASDAgent achieves nearly 80% strategic consistency with human experts. Moreover, we show that synthetic data produced by ASDAgent effectively distills professional clinical knowledge into small language models (SLMs), significantly enhancing their therapeutic capabilities.

NeuroWise: A Multi-Agent LLM "Glass-Box" System for Practicing Double-Empathy Communication with Autistic Partners

The double empathy problem frames communication difficulties between neurodivergent and neurotypical individuals as arising from mutual misunderstanding, yet most interventions focus on autistic individuals. We present NeuroWise, a multi-agent LLM-based coaching system that supports neurotypical users through stress visualization, interpretation of internal experiences, and contextual guidance. In a between-subjects study (N=30), NeuroWise was rated as helpful by all participants and showed a significant condition-time effect on deficit-based attributions (p=0.02): NeuroWise users reduced deficit framing, while baseline users shifted toward blaming autistic "deficits" after difficult interactions. NeuroWise users also completed conversations more efficiently (37% fewer turns, p=0.03). These findings suggest that AI-based interpretation can support attributional change by helping users recognize communication challenges as mutual.

ComPASS: Towards Personalized Agentic Social Support via Tool-Augmented Companionship

Developing compassionate interactive systems requires agents to not only understand user emotions but also provide diverse, substantive support. While recent works explore empathetic dialogue generation, they remain limited in response form and content, struggling to satisfy diverse needs across users and contexts. To address this, we explore empowering agents with external tools to execute diverse actions. Grounded in the psychological concept of "social support", this paradigm delivers substantive, human-like companionship. Specifically, we first design a dozen user-centric tools simulating various multimedia applications, which can cover different types of social support behaviors in human-agent interaction scenarios. We then construct ComPASS-Bench, the first personalized social support benchmark for LLM-based agents, via multi-step automated synthesis and manual refinement. Based on ComPASS-Bench, we further synthesize tool use records to fine-tune the Qwen3-8B model, yielding a task-specific ComPASS-Qwen. Comprehensive evaluations across two settings reveal that while the evaluated LLMs can generate valid tool-calling requests with high success rates, significant gaps remain in final response quality. Moreover, tool-augmented responses achieve better overall performance than directly producing conversational empathy. Notably, our trained ComPASS-Qwen demonstrates substantial improvements over its base model, achieving comparable performance to several large-scale models.
