Adaptive Conformal Prediction for Reliable and Explainable Medical Image Classification

Abstract

Deep learning models for medical imaging often exhibit overconfidence, creating safety risks in ambiguous diagnostic scenarios. While Conformal Prediction (CP) provides distribution-free statistical guarantees, standard methods such as Regularized Adaptive Prediction Sets (RAPS) optimize for average efficiency and can mask severe failures on difficult inputs. We propose an Adaptive Lambda Criterion for RAPS that minimizes the worst-case coverage violation across prediction set size strata. On OrganAMNIST (58,850 abdominal CT images, 11 classes), standard size-optimized RAPS converges to near-deterministic behavior with stratified undercoverage on uncertain samples, while our method achieves 95.72 percent global coverage with average set size 1.09 and at least 90 percent coverage across all strata. Cross-domain validation on PathMNIST (107,180 pathology images, 9 classes) confirms generalizability. Quantitative Grad-CAM analysis (rho = -0.30, p < 1e-22) shows that multi-label predictions correspond to focused attention on anatomically ambiguous regions. These results demonstrate that the proposed method improves reliability while maintaining efficiency, making it suitable for safety-critical medical AI applications.

ConRad: Efficient Conformal Prediction for Radiomics

Radiomic features derived from medical images and segmentation masks are used to support decision making in clinical imaging pipelines. In practice, these features are often computed from predicted masks, but segmentation models can be overconfident or poorly calibrated, making derived measurements appear more reliable than they are. Conformal prediction (CP) provides distribution-free prediction intervals with finite-sample marginal coverage guarantees, but black-box intervals for segmentation-derived radiomics can be inefficient because they ignore test-time information about image appearance, mask geometry, and segmentation uncertainty. We propose ConRad, a conformal framework for scalar radiomic targets that uses covariates derived from the predicted mask, input image, predicted radiomics, and boundary uncertainty to construct adaptive intervals while maintaining coverage. Across five 2D medical imaging datasets and 171 retained radiomic targets, we show that ConRad improves feature-level efficiency compared to baselines while maintaining near-nominal empirical coverage. Ablation results further indicate that segmentation boundary uncertainty features are the largest contributors to interval efficiency.

Cluster Frequency Conformal Prediction for Local Coverage

Conformal prediction provides distribution-free coverage guarantees, but in many-class classification it may still under-cover specific classes or subpopulations, preventing safe deployment in high-stakes applications. We propose Cluster Frequency Conformal Prediction (CFCP), a plug-in framework that adapts conformal prediction to local structure in a learned representation space. CFCP clusters learned embeddings, estimates cluster-level label-frequency distributions from calibration data, and for each test point constructs a sample-specific probability vector by softly mixing nearby cluster distributions regularized with global-prior and reliability-aware shrinkage. This vector is then conformalized using standard set constructors. In the disjoint-split regime, CFCP inherits standard finite-sample marginal validity. Under additional assumptions, CFCP further admits a local-validity interpretation. Since representation clusters aggregate locally similar samples, their empirical class frequencies provide a stable estimate of local label ambiguity. Across image and text benchmarks, CFCP achieves the best class coverage in 15/16 dataset/score-family comparisons and a competitive prediction set size efficiency, with several settings substantially more efficient. Overall, our results show that cluster-frequency information provides an effective localized signal for improving classwise reliability in many-class conformal prediction.

Adaptive Cumulative Mass Calibration with Conformal Prediction

Reliable probability estimates by classifiers are essential in high-risk applications. In practice, however, predicted probabilities are often miscalibrated, and many existing post-hoc calibration methods typically lack guarantees that a specific notion of calibration is achieved after the correction procedure is applied. We introduce a set-based perspective on calibration through the notion of cumulative mass calibration and the corresponding error measures. We propose a new calibration procedure based on conformal prediction that forms cumulative probabilities with guaranteed marginal coverage. We introduce an adaptive temperature scaling algorithm, with the temperature tuned for each input to satisfy the conformal coverage constraint. As we show, this procedure can be efficiently implemented. Across image classification tasks, particularly in settings with many classes, our method improves newly introduced calibration error measures (CMCE and αα-CMCE) and standard metrics (such as ECE, cw-ECE, MCE) over the existing baselines.
