Agentic AI for Personalized Physiotherapy: A Multi-Agent Framework for Generative Video Training and Real-Time Pose Correction

Abstract

At-home physiotherapy compliance remains critically low due to a lack of personalized supervision and dynamic feedback. Existing digital health solutions rely on static, pre-recorded video libraries or generic 3D avatars that fail to account for a patient's specific injury limitations or home environment. In this paper, we propose a novel Multi-Agent System (MAS) architecture that leverages Generative AI and computer vision to close the tele-rehabilitation loop. Our framework consists of four specialized micro-agents: a Clinical Extraction Agent that parses unstructured medical notes into kinematic constraints; a Video Synthesis Agent that utilizes foundational video generation models to create personalized, patient-specific exercise videos; a Vision Processing Agent for real-time pose estimation; and a Diagnostic Feedback Agent that issues corrective instructions. We present the system architecture, detail the prototype pipeline using Large Language Models and MediaPipe, and outline our clinical evaluation plan. This work demonstrates the feasibility of combining generative media with agentic autonomous decision-making to scale personalized patient care safely and effectively.

PhysAgent: Reflective Agentic Physics Control for Physically Plausible Video Generation

Recent advances in physics-grounded video generation leverage physics simulation as a physical prior to guide video synthesis toward physically plausible outcomes. The simulation process is controlled by physical specifications, which are typically generated by a vision-language model in a single pass. Such one-shot prediction often fails to accurately translate user intent into executable simulations, particularly for fine-grained object dynamics, complex motion trajectories, and temporally structured interactions. In this paper, we propose PhysAgent, a reflective agentic framework that closes the loop among physical program generation, physics simulation, stage-specific verification, and targeted program repair. Beyond improving the control of coupled physical parameters, our framework enables the agent to progressively realize complex trajectories, multi-stage interactions, and precise event outcomes by treating each physical program as an executable hypothesis. In addition, we design a set of physics-control APIs to support more stable and complex motion behaviors. Extensive experiments demonstrate that PhysAgent produces more physically plausible videos, achieves better prompt alignment, and generalizes more effectively across diverse physical scenarios.

PhysAgent: Automating Physics-Based 4D Synthesis via Trajectory-Grounded Multi-Agent Feedback

Achieving fully automated, physically plausible 3D motion synthesis is a core objective in graphics and generative AI. However, configuring complex environmental force fields still relies entirely on manual expert intervention, creating a severe bottleneck for large-scale simulation data generation. Existing automated methods primarily focus on material optimization and exhibit severe modality gaps and technical flaws when applied to the vastly more complex force field optimization space: naive Large Language Models (LLMs) lack underlying simulation feedback, causing severe physical inaccuracies, while traditional Score Distillation Sampling (SDS) suffers from sluggish gradients, local optima entrapment, and a mathematical inability to dynamically switch discrete force fields. To address this, we propose PhysAgent, the first simulator-in-the-loop multi-agent framework that leverages multimodal inputs for automated, physically grounded 4D synthesis. By decoupling intrinsic materials from extrinsic dynamics, PhysAgent utilizes a Semantic Agent equipped with an externalized Force Field Skill module to master simulation rules and generate valid initializations. Subsequently, the Refine Agents, driven by Trajectory-Grounded Multi-Agent Feedback, leverage vision foundation models to extract dense point trajectories from rendered frames. By converting these explicit motion trajectories into structured textual descriptors, the agent harnesses LLM commonsense reasoning to execute zero-shot macroscopic leaps, effectively escaping local optima and dynamically switching discrete force fields. Extensive experiments demonstrate that PhysAgent rapidly generates stable, diverse physical scenes from arbitrary multimodal prompts, significantly outperforming existing baselines in both generation diversity and physical accuracy.

VISTA: A Generative Egocentric Video Framework for Daily Assistance

Training AI agents to proactively assist humans in daily activities, from routine household tasks to urgent safety situations, requires large-scale visual data. However, capturing such scenarios in the real world is often difficult, costly, or unsafe, and physics-based simulators lack the visual fidelity needed to transfer learned behaviors to real settings. Therefore, we introduce VISTA, a video synthesis system that produces high-fidelity egocentric videos as training and evaluation data for AI agents. VISTA employs a 5-step script generation pipeline with causal reverse reasoning to create diverse, logically grounded intervention modes. These scenarios span two levels of agent autonomy: reactive and proactive. In reactive modes, the user explicitly asks the agent for help. In proactive modes, the agent offers help without receiving a direct request. We further divide proactive modes into explicit and implicit types. In explicit proactive scenarios, the user is aware of needing help but does not directly address the agent. In implicit proactive scenarios, the agent intervenes before the user even realizes that help is needed. VISTA allows users to customize and refine scenarios to generate video benchmarks for daily tasks, offering a scalable and controllable alternative to real-world data collection for training and evaluating AI agents in realistic environments.
