Abstract

Androgenetic alopecia (AGA) is characterized by patterned follicular miniaturization, increased single-hair follicular units, and altered hair-shaft diameter. We present an automated quantitative scalp-analysis and clinical decision-support framework combining FU localization, ordinal visible-shaft counting, calibrated shaft-width estimation, regional aggregation, and an interpretable rule layer. The clinical cohort comprised 243 patients (127 AGA, 116 non-AGA), while the computer-vision experiments used 160 expert-annotated patients, 2,400 trichoscopic images, and approximately 158,000 FU annotations. Under patientdisjoint evaluation, YOLOv8m achieved test [email protected]=0.920 and recall=0.860; EfficientNet-B5 with a support-map channel achieved 87.0% expert-box count accuracy (macro F1=0.85). A separate 500-image set was processed end-to-end with detector-generated boxes, yielding MAE of 6.56 for follicle detection and 16.59 for follicle classification relative to human-expert annotations. The system is intended to assist, rather than replace, dermatologist interpretation.

CG-HAF: An Interpretable Global-Local Lesion-Burden Fusion Framework for Ordinal Acne Severity Grading in Agentic Skincare Support

Ordinal acne severity grading requires distinguishing visually similar neighboring grades while jointly weighing holistic facial appearance and localized lesion burden - evidence that most existing approaches collapse into a single opaque representation. We introduce CG-HAF, a global-local fusion framework that instead keeps this evidence explicit: averaged holistic severity probabilities from independently trained classifiers are combined with structured lesion-burden descriptors from an object detector (lesion count, detection confidence, lesion area) into a compact representation, from which a lightweight, interpretable classifier produces the final grade. On a widely used benchmark, this fusion yields a clear, statistically supported improvement over global-evidence-only baselines, with the largest gains on the most severe cases. Testing on an independent dataset with a different grading standard shows that strong within-dataset performance does not transfer automatically, and a follow-up diagnostic attributes much of this gap to mismatched grading criteria rather than detection failure alone. These findings support interpretable global-local fusion as an effective strategy for ordinal acne grading while highlighting criterion alignment as key to cross-dataset portability, with a further illustration of how the resulting severity signal can support transparent, non-diagnostic decision-making in skincare applications.

Clinical Validation of the Melanoscope AI Mobile Dermoscopy Clinical Decision Support System

Introduction. Early detection of malignant skin lesions is critical for prognosis, yet dermatologist shortages in Russian regions limit screening coverage. Mobile dermoscopy clinical decision support systems (CDSS) offer a promising approach, with model interpretability and standardised patient routing remaining key barriers to adoption. Aim. To develop a quantitative interpretability assessment method for cascade deep learning models and a three-zone patient routing algorithm, and to conduct a preliminary single-centre prospective clinical validation of the Melanoscope AI CDSS in Russian outpatient practice. Material and methods. Two-stage cascade classification of dermoscopic images; attention map visualisation (attention rollout for ViT and Swin; Grad-CAM for ConvNeXt and EfficientNetV2); quantitative IoU-based agreement assessment between activation maps and expert annotations; prospective single-centre validation across four "Melanoma Day" sessions (Orel, Russia, June 2025 - April 2026). Results. On 176 patients: agreement with expert assessment 88.6%; no false negatives among 5 malignant lesions (95% CI: 47.8-100.0%); specificity 88.3%. Three melanomas and two basal cell carcinomas were histologically confirmed; six dysplastic naevi placed under follow-up. Mean IoU (n=180): ViT - 0.69; Swin - 0.64; ConvNeXt - 0.53; EfficientNetV2 - 0.51. Routing thresholds: P<0.15 / 0.15-0.50 / >=0.50. Conclusion. No false negatives were observed; specificity was 88.3%, supporting screening use. The integrated cascade classification, attention map visualisation with IoU assessment, and three-zone routing provide reproducible, interpretable clinical decision support adaptable to varying resource levels.

Retinal OCTA Phenotyping with LLM Reporting for Alzheimer's Disease

Early identification of Alzheimer's disease (AD) remains challenging because established assessment methods can be costly, resource-intensive, or unsuitable for population-scale screening. Optical coherence tomography angiography (OCTA) provides non-invasive visualization of retinal microvasculature, but existing approaches often require diagnostic labels and provide limited measurement-level interpretation. We present an explainable OCTA pipeline that integrates annotation-aware vessel segmentation, layer-specific vascular biomarker extraction, label-free phenotyping, and measurement-grounded LLM reporting. Using 117 ROSE-1 images from 39 subjects, we apply annotation-matched segmentation models to superficial vascular complex (SVC), deep vascular complex (DVC), and combined SVC+DVC representations. The models achieve ROC-AUC values of 0.916-0.970 and Dice scores of 0.695-0.781. Six density and fractal-dimension biomarkers form subject-level profiles for exploratory clustering. Analysis of nine held-out subjects identifies an internally consistent lower-density, lower-fractal-dimension phenotype, although the absence of diagnostic labels prevents clinical interpretation. Reports generated using GPT, Gemini, and Llama are evaluated for measurement grounding, citation faithfulness, and diagnostic caution. Overall, the framework provides a transparent, non-diagnostic connection between retinal vascular measurements, exploratory phenotyping, and evidence-linked interpretation for Alzheimer's research.
